By checking this box and proceeding with this request, I authorize WEHCO Video, Inc., and it’s operating affiliates Cam-Tel Company; Vicksburg Video, Inc.; Resort Television Cable Company, Inc; Hope Community TV, Inc.; Prescott Video, Inc.; Longview Cable Television Company, Inc.; Kilgore Video, Inc; Pine Bluff Cable Television, Inc.; East Arkansas Video, Inc.; White County Video, Inc.; Tahlequah Cable Television, Inc, (the Company) to transmit the information I provided in my Affordable Connectivity Program (ACP) application to the Universal Service Administrative Company (USAC) and the National Lifeline Accountability Database to verify my eligibility to enroll my household to receive benefits under the ACP, and to use and (to the extent necessary) transmit any information that I provide to third parties in connection with the administration of the ACP. I understand that my household may obtain broadband service supported by the ACP from any participating provider it chooses. I understand that my household may transfer its ACP benefit to another provider at any time. I understand that I must request to terminate my household’s ACP service by emailing the request to ACPOrder@wehco.com, calling 501-378-3529 or by mailing the request to WEHCO Video – ACP, PO Box 2221, Little Rock, AR 72203. I understand that I may change or cancel my service at any time during or after I am enrolled in the ACP. If I apply my ACP benefit in conjunction with an extended service contract, I will not be charged any applicable Early Termination Fee (ETF) for the duration of the applicable term. I understand that only one ACP-supported service is permitted per household and certify that no other member of my household is receiving an ACP-supported service. I understand that I am responsible for any monthly charges billed to my account greater than ACP benefit amount and my service can be disconnected for non-payment as set forth in the rules of the ACP. I certify that the information that I provided in my ACP application is true to the best of my knowledge. I provide affirmative consent to the Company to obtain the ACP subsidy on my behalf and to apply the proceeds to my household’s account for the service my household receives from the Company. I understand that the ACP is a government funded program and that the benefit may expire when funding is fully expended. I opt-in and agree that I will be subject to the Company’s undiscounted rates and general terms and conditions at the end of the program unless and until I later notify the Company of my desire to cancel the service. By requesting the ACP benefit through this form, I understand that I am consenting to apply the ACP benefit to my qualifying services, and authorizing the Company to transfer my ACP benefit from another provider if I am already enrolled in the ACP. By transferring the benefit, I understand that the ACP benefit will no longer be applied to the services received from another provider and I may be subject to the other provider’s undiscounted rates if I choose to continue service with that provider. I understand that my household is limited to one ACP transfer per calendar month with limited exceptions. I understand that If I decide to apply the ACP benefit to services received from another service provider, I will no longer receive the benefit on Cablelynx broadband service. Any benefit posted to my account during the month of transfer will be reversed and I will be responsible for full payment at then-current rates.
